Begin by contacting My Aged Care at 1800 200 422 or through their online platform. Initially, you’ll be screened for eligibility. If eligible, a Home Support Assessor will assess your needs at home and potentially refer you for an ACAT assessment for more comprehensive support. Upon approval, you’ll receive a notification of your package level and be placed on a national waitlist. When your package is ready, you’ll receive an assignment letter and will have 56 days to establish a Home Care Agreement with a provider of your choice.

Home Care Packages are an essential component of Australia’s aged care system, designed to support older Australians who wish to continue living independently in their own homes. These packages provide a coordinated and flexible approach to delivering care services tailored to individual needs.

Home Care Packages offer four levels of support, ranging from basic care needs to high-level care:

  1. Level 1: Basic care needs
  2. Level 2: Low-level care needs
  3. Level 3: Intermediate care needs
  4. Level 4: High-level care needs

Each level comes with a different funding amount, allowing for varying degrees of support based on the individual’s assessed requirements.

Eligibility Criteria

To be eligible for a Home Care Package, you must meet the following criteria:

  • Be an older person who needs coordinated services to help you stay at home
  • Be a younger person with a disability, dementia, or other special care needs that are not met through other specialist services

Additionally, you must:

  • Be living in Australia and have Australian citizenship or permanent residency
  • Be assessed and approved as eligible by an Aged Care Assessment Team (ACAT)

It’s important to note that eligibility doesn’t guarantee immediate access to a package, as there may be waiting periods depending on the level of care required and availability in your area.

Application Process

Step 1: Request an Assessment

The first step in applying for a Home Care Package is to request an assessment. You can do this by:

  1. Calling the My Aged Care contact centre on 1800 200 422
  2. Applying online through the My Aged Care website
  3. Having a health professional or family member refer you to My Aged Care

During this initial contact, you’ll be asked a series of questions to determine your care needs and eligibility for an assessment.

Step 2: Home Support Assessment

If you’re deemed eligible for further assessment, a Home Support Assessor will visit you at home. This assessor will:

  • Discuss your current situation and needs
  • Identify your goals and preferences for care
  • Determine if you would benefit from entry-level support services

If the assessor determines that you require more comprehensive support, they will refer you for an ACAT assessment.

Step 3: ACAT Assessment

An Aged Care Assessment Team (ACAT) member will conduct a comprehensive assessment of your care needs. This assessment typically involves:

  • A face-to-face meeting at your home
  • Discussion about your health, lifestyle, and care needs
  • Evaluation of your physical and cognitive abilities
  • Consideration of your social and emotional wellbeing

The ACAT assessor will then determine your eligibility for a Home Care Package and recommend an appropriate level of care.

Step 4: Approval and Wait List

If you’re approved for a Home Care Package, you’ll receive a letter outlining:

  • The level of Home Care Package you’re approved for
  • Your priority for receiving a package
  • Your unique referral code

You’ll then be placed on a national wait list. The wait time can vary depending on your assessed need and the availability of packages in your area.

Step 5: Assignment of a Home Care Package

When a suitable package becomes available, you’ll receive a letter of assignment. This letter will include:

  • The level of Home Care Package assigned to you
  • The date your package will start
  • Your unique referral code

You’ll then have 56 days to enter into a Home Care Agreement with your chosen provider.

Choosing a Service Provider

Selecting the right service provider is crucial for ensuring you receive the care that best meets your needs. Consider the following steps:

  1. Research providers in your area: Use the My Aged Care website or contact centre to find approved providers in your location.
  2. Compare services and costs: Request detailed information from several providers about their services, fees, and approach to care.
  3. Ask questions: Prepare a list of questions to ask potential providers, such as:
    • What specific services do you offer?
    • How do you tailor care plans to individual needs?
    • What are your staff qualifications and experience?
    • How do you handle complaints or concerns?
  4. Check quality ratings: Review the quality ratings of providers on the My Aged Care website.
  5. Seek recommendations: Ask friends, family, or healthcare professionals for recommendations based on their experiences.
  6. Consider cultural fit: Look for providers who can cater to your cultural, linguistic, or religious needs if applicable.
  7. Evaluate flexibility: Choose a provider who can adapt their services as your needs change over time.

Understanding Funding and Fees

Home Care Packages are subsidised by the Australian Government, but you may be required to contribute to the cost of your care if you can afford to do so.

Government Subsidy

The government subsidy for Home Care Packages varies depending on the package level:

  • Level 1: Approximately $9,000 per year
  • Level 2: Approximately $15,750 per year
  • Level 3: Approximately $34,250 per year
  • Level 4: Approximately $52,000 per year

These amounts are subject to change and may be adjusted annually.

User Contributions

There are two types of fees you may be asked to pay:

  1. Basic Daily Fee: This is set at 17.5% of the single basic Age Pension and is the same for all package levels.
  2. Income-Tested Care Fee: This is an additional contribution that some people may need to pay if their income is above a certain threshold. The Department of Human Services will assess your income to determine if you need to pay this fee and how much.

It’s important to note that there are annual and lifetime caps on the income-tested care fee to protect those who need care for extended periods.

  • Includes a budget for how your package funds will be spent

Reviewing and Adjusting Your Care

Regularly review your care plan with your provider to ensure it continues to meet your needs. You can request changes to your services at any time if your circumstances or preferences change.

Understanding Your Monthly Statement

Your provider should give you a monthly statement that shows:

  • The government subsidy amount
  • Your contribution amount
  • How your package funds have been spent
  • Any unspent funds

Review this statement carefully and ask your provider to explain any items you don’t understand.

Handling Unspent Funds

Any unspent funds in your package remain yours and can be used for future care needs. If you change providers, these unspent funds will transfer with you.

Quality Standards and Compliance

All Home Care Package providers must comply with the Aged Care Quality Standards set by the Australian Government. These standards cover areas such as:

  • Consumer dignity and choice
  • Ongoing assessment and planning with consumers
  • Personal care and clinical care
  • Services and supports for daily living
  • Organisation’s service environment
  • Feedback and complaints
  • Human resources
  • Organisational governance

As a care recipient, you have the right to receive high-quality care that meets these standards. If you have concerns about the quality of care you’re receiving, you can:

  1. Discuss the issue with your provider
  2. Contact the Aged Care Quality and Safety Commission
  3. Change providers if you’re unhappy with the care you’re receiving

Rights and Responsibilities

As a Home Care Package recipient, you have both rights and responsibilities.

Your Rights

You have the right to:

  • Be treated with respect
  • Receive safe and high-quality care and services
  • Be informed about your care and services in a way you understand
  • Have control over and make choices about your care, personal and social life, including where the choices involve personal risk
  • Have control over, and make decisions about, the personal aspects of your daily life, financial affairs and possessions
  • Your independence
  • Be listened to and understood
  • Have a person of your choice, including an aged care advocate, support you or speak on your behalf
  • Complain free from reprisal, and to have your complaints dealt with fairly and promptly
  • Personal privacy and to have your personal information protected
  • Exercise your rights without it adversely affecting the way you are treated

Your Responsibilities

You have the responsibility to:

  • Respect the rights of care workers to their human, legal and workplace rights including the right to work in a safe environment
  • Treat care workers without exploitation, abuse, discrimination or harassment
  • Tell your provider about any problems with the care or services
  • Pay any fees outlined in your Home Care Agreement
  • Tell your provider about any changes to your circumstances that may affect your care needs or your ability to pay fees

By understanding and fulfilling these responsibilities, you can contribute to a positive and effective care relationship with your provider.
